The Accuracy Ledger

Every grade Flow issues in a seat where the outcome can land in public — with its evidence, and the outcome appended when it lands. One-shot API checks are never entered here: their outcomes happen inside the caller's flow, where we can't see them, and claiming accuracy on outcomes we can't observe is exactly the kind of number this ledger exists to refuse.
